Recommendation Form
Please complete the counselor recommendation form (PDF) and mail it to the University of Dayton with:
- An official transcript of courses taken and grades received.
- Results of either the ACT or SAT.
- The current senior-year courses in which the student is enrolled and grades for any courses completed.
